What to Bring

This is the list of documents you will need to prepare and bring to your appointment. Please read carefully so you will be prepared at the time of your appointment.

Please bring the following...

 

New Application:

Follow these instructions if one or more of the following applies to the applying person(s):

  • The applicant is 17 years old or younger
  • The applicant has never owned a passport before
  • The applicant has had a passport before but it was a minor passport (a passport issued to someone 15-years or younger that is valid for 5 years)
  • The applicant has had a passport before but it was issued over 15 years ago
    • Please Note: If an applicant is 15-years-old or younger, BOTH parents/guardians listed on their birth certificate MUST be present at their appointment.  
    • Please Note: If an applicant is 16 or 17-years-old, ONE parent/guardian listed on their birth certificate MUST accompany them to their appointment.

 

  1. COMPLETED DS-11 Form
    • Application must be printed on TWO SEPERATE pages (not double sided)
    • Application must be fully filled out with BLACK INK only
    • Application must be free of corrections/cross outs/ white out (if you make a mistake start over on a fresh application) 
    • Application must NOT be signed until asked to do so by an agent
  2. Primary Evidence of U.S. Citizenship (One of the following)
    • Birth certificate with original seal (IF APPLICANT IS UNDER 18 A BIRTH CERTIFICATE IS REQUIRED TO APPLY)
    • Most recent passport (undamaged)
    • Naturalization document with original seal
  3. One Form of Primary Identification with Photo (ANYONE AGE 16 AND OLDER MUST PRESENT A VALID ID)
    • Valid Driver's License
    • Valid State ID Card
    • Current Government ID (city, state, federal or military)
    • Valid previously issued, undamaged U.S. passport
    • Naturalization Certificate
    • Student ID
  4. Payment 
  5. Name Change Documents (Disregard this step if your name has not changed since birth)
    • Marriage License (ONLY if your name has changed due to marriage)
    • Name change court documents (ONLY if your name has changed due to personal circumstance)
  6. Two Passport Photos (OPTIONAL)
    • The UNL Passport Office offers photo services for $15.00 per person, photos are taken during appointments. 
    • You may bring in pre-taken photos if it is your preference. If you choose to do so, the pictures MUST meet these requirements: Photo Quality Requirements


Passport Application Fees

 

Renewal Application: 

Follow these instructions if the following applies to the applying person(s):

  • The applicant has had an adult passport (a passport issued to someone 16-years or older that is valid for 10 years)
  • The afore mentioned passport was issued within the last 15 years
  • The afore mentioned passport is NOT voided (free of hole punches)
  • The afore mentioned passport is in good condition (no rips, water damage, or damaged front cover)

 

  1. COMPLETED DS-82 Form
    • Application must be printed on TWO SEPERATE pages (not double sided)
    • Application must be fully filled out with BLACK INK only
    • Application must be free of corrections/cross outs/ white out (if you make a mistake start over on a fresh application) 
  2. Old Passport
    • Passport must have been a 10-year passport (valid for 10 years)
    • Passport must have been issued in the last 15 years
    • Passport has not been voided (free of hole punches)
  3. Payment
  4. Name Change Documents (Disregard this step if your name has not changed since the issuance of your most recent passport)
    • Marriage License (ONLY if your name has changed due to marriage since the issuance of your last passport)
    • Name change court documents (ONLY if your name has changed due to personal circumstance since the issuance of your last passport)
