What to Bring for your Passport Application

Out of concern for the health and safety of our campus community, the UNL Passport Office is suspending services.

It is unknown at this time when the UNL Passport Office will resume operations. If you are in immediate need of obtaining a passport, please visit travel.state.gov.

Here is a list of documents that you will need to prepare before your visit to the UNL Passport Services office. Please go through each item carefully. The Passport Office does not accept any type of bank (credit/debit) or prepaid cards.

You can also use the travel.state.gov passport wizard program to get your documents ready.

Documents to Prepare

  1. COMPLETED Form DS-11
    • Do not sign the form until at the UNL Passport Office
  2. Primary Evidence of U.S. Citizenship (One of the following)
    • Certified birth certificate issued by the city, county or state
    • Valid previously issued, undamaged U.S. passport
    • Naturalization Certificate
    • Certificate of Citizenship
  3. Primary Identification (One of the following)
    • Valid Driver's License
    • Current Government ID (city, state or federal)
    • Current Military ID (military and dependents)
    • Valid previously issued, undamaged U.S. passport
    • Naturalization Certificate
  4. Applicable Fee
    • The Application Fee will be paid to the U.S. State Department. The fee is included with your mailed application. Fee is payable using
      • check – personal or cashier
      • money order
    • NOTE The Acceptance Fee will be paid to the University of Nebraska–Lincoln. Fee is payable using:
      • check – personal or cashier
      • money order
  5. Optional: Two Passport Photos
    • View the Photo Quality Requirements
    • We can take your photos at the UNL Passport Office for $15.00. Your reservation time includes the taking and processing of photos.
